چکیده

INTRODUCTION G astroesophageal reflux disease (GERD) is extremely common and encompasses a diverse spectrum of clinical presentations. It has a substantial impact on patient quality of life and use of health care resources (1). GERD results from failure of the lower esophageal sphincter (LES) and cardia to prevent return of gastric contents into the esophagus. The pathophysiology of GERD is multifactorial and includes the occurrence of inappropriate, non-swallow related LES relaxations, and low resting pressure of the LES. This episodically exposes the esophageal body to gastric acid and enzymes (2). Reflux usually manifests as heartburn and regurgitation, and predisposes to the development of esophagitis, Barrett’s metaplasia, and esophageal adenocarcinoma (3). Current therapy for GERD begins with lifestyle changes and medical treatment, which prove to be adequate or effective in more than 95% of patients. Such therapy must usually be maintained long term as the recurrence rate of symptoms is as high as 90% after cessation of medication. In addition, long term drug therapy is associated with issues of cost, compliance, and long term safety (4). Patients who do not tolerate or respond adequately to medication or who wish to avoid life-long drug therapy may be considered surgical candidates. Such surgery necessitates general anesthesia, has a mortality of approximately 0.2%, and can be associated with A SPECIAL ARTICLE

Endoscopic Treatment of Refractory Gastroesohageal Reflux Disease

Though efficient acid suppression with proton pump inhibitors (PPIs) remains the mainstay of treatment of gastroesophageal reflux disease (GERD), some of the patients showed refractory response to PPIs, necessitating further intervention. After increasing dose of PPIs and other kinds of pharmacological intervention adopting prokinetics or others, variable endoscopic treatments are introduced fo...

A preliminary investigation of anti-reflux intervention for gastroesophageal reflux related childhood-to-adult persistent asthma

BACKGROUND Childhood-to-adult persistent asthma is usually considered to be an atopic disease. However gastroesophageal reflux may also play an important role in this phenotype of asthma, especially when it is refractory to pulmonary medicine. METHODS Fifty-seven consecutive GERD patients who had decades of childhood-to-adult persistent asthmatic symptoms refractory to pulmonary medication we...

Transoral Incisionless Fundoplication for Refractory Gastroesophageal Reflux Disease: Where Do We Stand?

Gastroesophageal reflux disease (GERD) is a chronic, progressive, and costly medical condition affecting a substantial proportion of the world population, predominantly the Western population. The available treatment options for patients with refractory GERD symptoms are limited to either laparoscopic surgery with significant sequelae or potentially lifelong, high-dose proton pump inhibitor the...

Long-term outcomes of patients with refractory gastroesophageal reflux disease following a minimally invasive endoscopic procedure: a prospective observational study

BACKGROUND Gastroesophageal reflux disease (GERD) is the most common digestive disease, affecting one third of the world's population. The minimally invasive endoscopic Stretta procedure is being increasingly used as an alternative strategy to manage refractory GERD. However, long-term benefits of this procedure have to be further evaluated in clinical settings. This prospective observational s...
